Applying for a Scholarship
Applications are accepted on a rolling basis. Please review the eligibility and requirements below before applying. The application and all required documentation must be submitted for the application to be considered.
If applying online creates a barrier, please contact us at info@communityhealthwmt.org or 406.926.2522 to request an application. Requested applications and required materials must be returned to Community Health Alliance at 2315 McDonald Ave Ste 102, Missoula, MT 59801. It is the responsibility of the applicant to ensure all materials are received.
Eligibility & Requirements
To be eligible to apply for a scholarship, applicants must:
- Be currently employed in the health field in Western Montana
- Work an average of 20 or more hours per week
- Have completed at least 6 months of employment (12 months preferred)
- Be a Montana resident
- Be pursuing skill-based training, post-secondary education, or certification/licensure for an industry-recognized credential
- Use funding for eligible expenses such as tuition, registration or certification fees, or travel costs related to the opportunity
Applicants must provide:
- A description of the educational or training opportunity
- An explanation of their motivation for pursuing the opportunity
- A summary of their professional goals
- A statement on how the opportunity will help improve health outcomes in Western Montana
- Information about their past and current healthcare employment
- Authorization from their supervisor and employing entity.*
*Self-employed individuals may submit the supervisor and entity authorization themselves.
Selection & Payment
Applications are reviewed and award decisions are made by the Education Committee of our Board of Trustees. Award notifications are generally provided within 45 days of application submission.
Awards are paid directly to the vendor whenever possible. Receipt of payment is required for reimbursements.
