Grant Cycles
Two cycles are available per year.
Spring: Our 2026 spring cycle will open on March 17. Applications are due May 13 and award decisions will be made by June 15. Award categories available in the spring cycle include:
- Rehabilitation Services and Education: Funded by the Gavin Family Endowment to support quality rehabilitation services and education that restores health for western Montanans.
- Total funding available: $35,000 | Max grant: $10,000
- Maternal Health: Funded by the Spierling Obstetrics Endowment for programs and services that support the health and well-being of mothers and infants during the prenatal and postpartum period.
- Total funding available: $10,000 | Max grant: $5,000
Fall: Our 2026 fall cycle opens mid-September with a December award date. Award categories will be determined at a later date, but generally include Rehabilitation Services and Education, and Diabetes Education.

Eligibility Criteria
Community Health Grants are open to charitable nonprofit entities, not private foundations, with tax exempt status under Section 501(c)(3) of the IRS code, and where appropriate, under Section 170 (c); or educational institutions.
Applicants must be applying to address a health challenge, need or opportunity that meets the mission of Community Health Alliance and the purpose of the funding category for which you intend to apply. Preferably, applicant organizations serve children, families, and vulnerable populations in Western Montana, offer unduplicated services, and who, through collaboration, are able to increase the impact of any Community Health Alliance funding received.
Community Health Alliance will not accept applications:
- From organizations that discriminate against race, color, religion, sex (including pregnancy, gender identity, and sexual orientation), national origin, disability, age, or genetic information.
- From private foundations.
- For solely individual benefit.
- To support fundraising events.
- To support medical research.
- To influence elections.
- To support projects for the sole purpose of data collection.
Applicants can apply once per year per category, and may submit one application per cycle.
Grant Process
- Complete an Application – Fill out our application form, available online or by contacting our office, and provide the required information and documentation by the cycle deadline. Applicants can apply once per year per category, and may submit one application per cycle.
- Committee Review – Applications are reviewed following the deadline. Only complete and qualified applications are reviewed by the committee.
- Notifications Sent – Applicant organizations will receive a notification no later than the 15th of the month following the cycle deadline:
- Spring cycle – notifications sent by June 15
- Fall cycle – notifications sent by December 15
*Organizations receiving a grant award will be required to complete and submit a signed grant addendum.
- Award Payment – Upon receipt of the signed grant addendum, the organization will receive grant payment directly from Community Health Alliance via check.
- Grant Report – Recipient organizations are required to submit a final report. Reports are due within one year of the grant award date. Grant reports can be submitted HERE.
- Site Visits – Site visits may be requested by our committee. If a site visit is requested, our staff will connect with your organization following the submission of your final report.

Grant Reports
Organizations receiving a community health grant are required to submit a final report. Reports are due within one year of the grant award date. Please submit your final report HERE.
